Title: Program Manager, New York City Health + Hospitals

Raised in East Hampton, NY, Christin Aucapina has called New York City home since arriving to complete her Master’s in Public Health at Columbia University. She has proven to be a dedicated member of the health community ever since. Since joining New York City Health + Hospitals’ Special Populations Program in 2021, Christin has grown from overseeing the operations of one clinic to supporting all six Special Populations CHW teams across five public hospitals. As the Project Manager for the Special Populations Community Health Worker (CHW) Program, Christin supports CHWs in Safety Net and Point of Re-entry and Transition (PORT) primary care clinics. The Safety Net and PORT clinics aim to serve chronically ill patients who are unhoused or who are reentering the community. Christin supports the refinement of the program by providing technical assistance and creating coaching tools to assist CHWs and CHW supervisors.
